Passport Rehabilitation Program


National HealthCare Associate's goal is to achieve the best possible rehabilitative results in the shortest time possible. NHCA strives to return patients to their highest level of functioning and, if possible, to independent living. These services normally follow an acute hospital stay.

NHCA specializes in providing rehabilitation services related to the following conditions:

  • Wound and decubitus management

Long Term Care (Skilled Nursing)


For more than 20 years, National HealthCare Associates (NHCA) has been recognized as a leader in the Long Term Care industry. Our Medicare and Medicaid certified facilities offer the highest levels of quality care with respect for each resident living in one of NHCA's 25 locations in Connecticut, New Jersey, New York, and Vermont.

Palliative Care & Hospice Services Coordination


Working in conjunction with certified Hospice care providers, NHCA offers a wide range of clinical and supportive services aimed at providing comfort and dignity to individuals with terminal illnesses. Services offered within NHCA's palliative care model include:
  • 24 Hour Skilled Nursing Care
  • Medication Management
  • Aromatherapy
  • Spiritual and Emotional support
  • Recreational Programs, as appropriate
  • Family Counseling and Care participation

Dementia/Alzheimer's Programs

NHCA offers specialized services to meet the needs of individuals with a wide variety of dementia related disorders, including Alzheimer's disease. Many of NHCA's facilities provide state of the art facility programs featuring:

Respite Care

Caring for someone in need of medical and emotional support requires tremendous energy. For those times when a caregiver needs some relief, whether for a few days or a couple of weeks, NHCA offers respite services. An individual normally cared for at home or similar setting can be admitted to a NHCA facility for a short term stay in order to provide the family caregivers a chance to recover from the stresses of daily care, take a vacation or focus on other personal needs. Services offered in conjunction with a respite stay include:
  • 24 Hour Skilled Nursing Care, as needed
  • Medication Management
  • Recreation programs
  • Physical and/or Occupational Therapy, if indicated
  • Nutritional needs management
  • Family support counseling
